It depends on what school and department you are applying to. The grad school has their requirements, but the department will have additional requirements.
GPA has to be atleast 3.0/4.0 scale
Min GARE 1100-1200
Min 3 Recommendations
Research experience esp if going for a PhD.
If you a writing a thesis or dissertation you should be able to get a fellowship from the respective department.
