does anyone know a site where i can find 2 record-keeping forms found at early childhood centers?

i need a sample of an accidental report and an emergency form. its for my CDA class.

I would suggest asking at a preschool or day care center. They should be very willing to give you a sample, blank form.

I’m not sure what you need for your emergency form–but here are a few sites that I have found that offer a variety of forms that can be downloaded.

http://www.childcarelounge.com/printform/ACCIDENT%20REPORT.doc –This site has an accident form that you can download for free.

http://www.geocities.com/dormanfcc/Forms.html- Free forms that you can download and print off. This site has an incident report–which may work for your emergency form. They also have a medication form as well. I’m not sure what you need for you emergency form.

I hope that these sites help you. I am a preschool teacher and I love it when you can find a site that you can print ideas, lessons, etc. for free–it’s a huge help.
