Comma before the ‘or’ , no ‘why’, but other changes are needed:
Is the reason you don’t want to be a friend to
[or ‘to be friends with’ – idiomatic] your cousin because you are holding a grudge against him, or because you don’t have anything in common?

I would write, “Is the reason you don’t want to be friends with your cousin because you are holding a grudge against him or because you don’t have anything in common.”
no comma before “or”.

i could write, “Is the reason you do no longer choose to be associates at the same time with your cousin considering you’re conserving a grudge against him or considering you haven’t any longer something in difficulty-loose.” no comma till now “or”.
