According to Merriam-Webster’s editors:
“The answer is simple: usage.”

Most other dictionaries will use a similar process.

Usually when a new dictionary (or new edition of an older dictionary) is published, the editors will include a preface that describes new features and how they go about selecting entries for the book. Look in the front of your dictionary!
