Where do I send this scholarship?

The United Association of Workers (UA) is doing a scholarship, and I did the scholarship, but I lost my log-in number and I can’t remember the address to mail it to. Can someone give me tha address? I know I mail it to Washington DC, but that’s it. Thanks alot!!!
