You have to be saying something insulting to someone who either has power or position over you.
Example: With all due respect, boss, your face and my *** bear a suspicious resemblance.
Or
With all due respect, Ma’am, that’s the worst idea since the zeppelin.
Another good one: no offense.
Any time some starts a sentence with ‘no offense’ you are about to be insulted.

Rules? Usually it is used when the person hold the other in low regard, so the respect due is questionable.

I don’t believe it’s a rule, but people use this phrase when what they want to say may be offensive. Yet, they truly feel that it must be said.

It’s merely a polite way of saying that you respect someone’s opinion before you give them yours or state a fact unbeknownst to the speaker.

i have used this phrase in terms of letting my principal know she was incorrect in accusing me of socializing on my job,aparticular teacher was expressing their stress to me i was only listening so i said to her”Mrs. ………with all due respect to you” i have been accused by you before of the same thing but you know iam not like that ,i keep a businesslike attitude
and if i need to go to a teacher it is about a student’s attendance and not to have fun.”
