My instructor gave me an evaluation for my internship at the clinic and I got rid of it?

I had a health unit coordinator internship last year at a major hospital in mn and it didn’t go so smoothly because I was stressed about problems going on outside of work so I feel that I would have done better if it wasn’t for that. When she wrote the eval she told me that she did not see me as a fit for the job but agreed that I could have done better. She explained that I needed to turn in a copy of the eval to have a position at that hospital. I ended up getting rid of the papers because I wasn’t proud of the internship. the question is, would my instructor have copies of the paperwork?

In most cases, the instructor will keep a copy of the evaluation for their own records. Especially if it was typed. It is very easy to keep files on your computer. I know I always keep copies of the evaluations I do for my interns, good or bad. It can’t her to ask her.
