11 year old with very poor number sense and low executive function. Lots of trouble with math .?

She is 11 years old and going into 5th grade. she repeated 3rd grade. She hates doing math and tries to get out of doing it. She is being tutored 2 hours a week right now. The tutors are good but, her memory for math is sporadic at best and she struggles with very simple facts. She does not have any logic behind her math skills. For instance, she knows that 6 times 5 is 30. But she doesn’t know that 6 times 6 is therefore not 27 or 89. She has lots of trouble with concepts like time, distance, temperature, measurement. And her general organizational skills are almost non-existent. She has been thoroughly tested (twice) and is intelligent and has no known delays but, how is that possible? Any info or advice would be appreciated!!
